School/Sports/Camp Physicals
Payment for sports physicals is required at the time of the visit. Most insurance carriers will not cover these evaluations. The fees for these physicals have been discounted so that more students can participate in the positive outlet and experience of camp, sports, and school involvement. These physicals cannot be billed to your insurance. At the time of your visit, please provide a physical evaluation form from the camp, team or school you plan to participate in.

Motor Vehicle Accidents
In order to evaluate you or treat you for injuries incurred by a motor vehicle accident, you will need to provide proof of auto insurance information at the time of your visit. The state of Oregon requires that we bill your insurance regardless of who is at-fault for the accident. Your insurance agent will handle the details regarding responsibility. Please be prepared to provide a copy of your insurance card or information, as well as a claim number.

Employer's Corner We are available for your employees six days a week. We will see your worker the day of the injury. No appointment necessary. We minimize insurance costs and claims costs by providing affordable services. (significantly less than the costs of a typical emergency room visit). We believe one of the best things to do for an injured worker is to return him to his normal work enviornment as soon as is safely possible. We can provide pre-employment or random drug screening. Our physician will go out to job sites, familiarizing themselves with the work enviornment and meeting with employers, supervisors and managers. Our staff can call the job site after visits to update the employer on employee status and return to work standing. We provide consistent delivery of care and consistent return to work guidelines.

Immigration Physicals
Because this physical requires a longer visit with the doctor, we request that you schedule an appointment for an Immigration Physical. You will need to return two days later to complete the evaluation. Please be sure to schedule your appointment accordingly. Please be prepared to provide Form I-693 from the US Department of Justice for the physician to fill out. You should bring with you any records of immunizations that you have. Also, please note that we require full payment for the physical at the time of visit. You may pay for your visit with cash or credit. We do not accept checks for immigration physicals. Immigration physicals cannot be billed to your insurance.


Drug Screens
These include pre-employment UAs, random drug screens, and at time pre-employment physicals which are pre-arranged by your employer through a special service.
